Host Home Services are provided in a private home by a principal care giver who lives in the home and either rents or owns the home. Host Home providers provide services under the supervision of a Residential Habilitation or Supported Living provider. American Health Care Services, Inc. is a Supported Living provider.

American Health Care Services, Inc. has 24-hour responsibility for arranging and overseeing the Host Home, providing emergency services as needed and providing or arranging for 14 days of respite per year. American Health Care Services Qualified Individual Developmental Specialist (QIDP), will coordinate the delivery of professional services to each person residing in a host home environment. Our host home program enables a person to retain or improve skills related to: community mobility; health; activities of daily living; money management; recreation; cooking; shopping; use of community resources; and safety.

Limits and Requirements
The total number of participants (including those served in the waiver) living in the home, who are unrelated to the principal care provider, cannot exceed three.

Host Home services refer to a residential arrangement in which a private homeowner provides room, board, personal supports and assistance to an unrelated person in is home for financial consideration agreed with an Agency. The services provided by a Host Home includes, but will not be limited to:

  • Room and board
  • Light homemaker tasks, such as assistance with meals preparation.
  • Light chore tasks, such as assistance with laundry, shopping, and general housekeeping.
  • General supervision of the person as described in the ISP and Plan of Care.
  • Assistance with personal hygiene.
  • Assistance with dressing.
  • Assistance with monitoring the person
